  • When there is a concern about repeat offending
  • When a child could be at risk of harm
  • When there could be a breach of FA Disciplinary Regulations 
  • If you have tried to address a matter but it continues to happen
  • Incidents involving a statutory agency, for example police, social services or a Local Authority Designated Officer
